Good Morning Parent Reps,

Please find attached the worksheet for calculating your team insurance cost and the invoice for annual league dues. In order to maintain insurance for each team, we require an initial roster by 6/1/2021. Our insurance renews on 6/3/2021 so it is a tight turnaround.

Insurance Instructions: 

❖ For both Initial Roster (due by 6/1) & Final Roster Additions (as applicable; due by 7/1): 

1. Mail/deliver this completed form with check/payment to: 
   1. RMSL Treasurer, 4790 S Franklin St., Englewood CO 80113
   2. Make check payable to Rocky Mountain Swim League
2. Email team rosters (Excel/spreadsheet format preferred) to: [email protected] 
3. Team roster should contain, preferably in spreadsheet format, the first name, last name, gender and age (as of June 1) for each swimmer and coach. Please condense to the minimum required information. 

We will determine the finals fee (a per swimmer fee) based on the number of swimmers in the league. We will have that number for the June RMSL meeting.
